Credit Card Authorization Form Examples

1. Under Administration on the left side, click Forms Manager
2. Turn on “All Reservations” under the form you have created
3. To inform the client what they will be paying change the field name to “Payment Amount” (filling this in will then send a popup to the agent that they will have to fill out prior to them sending it out.)
4. Hit the Save button and go to the Form Content tab.
5. To add an Acknowledgment box, click the title and click “Acknowledgment” (this checkbox will say that they are agreeing to being charged.)
6. You will have to check the box at the bottom to allow them to charge their credit cards.
7. Next add another form item and select “Plain Text Box” and type in “Amount to be charged is $”
8. Then from the merger tag drop down, select “Payment amount”

   Add another Plain Text Box and add some text explaining “By signing this I have read over my reservation…”

9. If you are wanting to add a link to your booking terms, Add a “HTML” box

   Then add another form item, “Signature Block” or Signature Tab so they can sign/type their name. And hit save.

10. Next go to Reservations – Current Reservations – a reservation – the forms button on top right and find the form you created to see what it looks like.

    When you hit the send button, it will popup with a price to type in.

11. If you have a specific card you want to be charged, go back into editing the form – then add another form item – credit card entry form.
